In order to put your wife's name on the deed you need to fill out a "quitclaim" form to add her to the deed. You may find them online.
Fill out quitclaim form completely.
Notarize quitclaim deed.
Take the form to your county recorders office to record the deed.
Some states might differ so call the county recorder first to be sure you have all the documentation they require.
