The legend of the "notch to be driven between teeth" is totally bogus and was never an authorized procedure. Tag, Identification, M-1940 - Stock No.74-T-60 of 15FEB1940 specifies: a notch at left to position Tag on embossing machine. FM10-63, Graves Registration, 15JAN1945: Tag will be attached to the remains when interred.

dog tags come from the domain of dogs who wore them long before we humans did,to simplify a soldiers identity the issue of i.d.tags was instituted,as they resembled a dog i.d,tag the nickname dog tags was born, each soldier has 3 tags,1 for his neck,1 for his leg,and one for his wrist in order to identify his body in the event of being blown up,dog tags have the soldiers number, rank, name and blood group embossed on them

I guess because they looked like dog tags. Originally metal disks embossed with personal information that could be left with a body on the field of battle for identification. Eventually it evolved into a rounded rectangle with a small indentation on one side so that it could be set on the teeth of a deceased soldier and kicked into the head so that the enemy could not strip the dead soldier of his identity (this info confirmed with HQMC Casualty Branch).
